Position: Chaplain & Medical Social Worker.
Chaplain shift is Monday thru Friday 8am-5pm
Primary Functions are to provide direct spiritual and emotional support to hospice patients and family members and to serve as a member of the interdisciplinary team.
Qualifications: Master of Divinity, appropriate endorsement within his/her domination, completion of some studies in Clinical Pastoral Education, two years of ministerial experience, ability to work effectively within an interdisciplinary team and across denominational/faith lines, flexible, current driver's license, on-call availability, computer skills and excellent communication skills.

Medical Social Worker shift is Monday-Friday 8am-5pm, On-call rotation is required.
Qualifications: BSW, 2 years Hospice/Home Health experience preferred, extensive computer skills, flexible, current dariver's license, on-call availability, excellent communication skills.
